You can also download the needed form in PDF format without creating a subscription.

The LIES the FRAUD the GAMES
So I’ll begin by saying you sign up under a name like free legal forms. Free. That word is used in silence. Much like I have, for example, a friend whose name is freelance but the free is silent. Same with this clickbait. Not only will they charge you an irate 39.99 a month but the best part is their customer service is only open from 2am til noon EST. Yup. While you’re asleep they’re playing games with those who are up and trying to resolve unauthorized charges. But they say they’ll refund you and days will pass but they don’t. They just lie. I despise companies such as this with their lackluster operating procedures. They have to trick you into your payment and then refuse to give it back unless you dispute your account and make your bank step into get it. Avoid these people you’ll be glad you did.

Free forms are available for downloading in PDF format, and they do not require any subscription. Subscription is needed to use full functionality of our service — online editor, filling-out guides, your own account with saved information, etc.
We offer multiple subscription options, including a monthly subscription with a 7-day free trial period that allows our customers to access our database of personal and business forms and templates, as well as online document editing software through our website. There is no charge if the subscription is canceled within the 7-day free trial.
There is also a way to cancel the subscription in your account directly. You can go to “Billing” in your profile menu and click on “Cancel Subscription” under your plan name.

WARNING
I created the account , paid $21 to be able to print the document I needed then there was a message error and the document disappeared. There was no data in my account. I wrote an email to the help email address they provided and message came back undeliverable.Recepient address rejected-domain not found. I guess FormsPal is a SCAM.

There are several ways to cancel your account during your free trial period:
1. In your account settings by entering “Billing Information” in the profile menu and clicking "Cancel Subscription".
2. By contacting our support via email or contact form. Our customer support team gets back to our clients regarding every request as soon as possible (usually within 24 hours).

Government forms can be accessed for free directly from the agency or department. This company just scams people out of money when they're too stressed or stupid to know better. The "5-star reviews" on this site (and other review sites) are clearly posted by the company.

Used this app for the trial period
Used this app for the trial period. Was charged a fee of $39.99 anyway.
Tried to log in to cancel and every page is blank, keeps telling me the email is incorrect even though the receipt for the withdrawal was sent to my email!
Called customer service, no answer after an hour! Not to mention customer service hours are 2AM-12PM EST!!!
DO NOT USE THIS SERVICE!

avoid at all costs! First, I was lured in by the "free" form ad. Filled out all the blanks and at the end it made me pay for a one-time $37 for the form I was requesting. Never received the form, called the customer support and sat on hold for 45 minutes. A total waste of time. Avoid, Avoid, Avoid. A scam
