Follow the simple instructions, pay the fee, and upload the forms needed by the school. Signed forms were returned via email within 24 hours.

I needed a sports clearance for my son. This platform was very easy to navigate from my phone. The doctor was very prompt. Will definitely use it again.

My Son needed a sports Physical for high school wrestling, his primary doctor was unable to see him before the season starts so I did a quick search online and found Dr. Kawalek, answered the questions online, uploaded my sons school physical form and had everything done and turned in within 24 hours. Very quick, easy and convenient.

SportSlip online services was wonderful and efficient especially being done in the comfort of your own home, and with the new school year starting and sports requiring a sports physical. This experience was easy on the teen and the parent. Same day service and results would allow the school of choice to be emailed the results of the sports physical, and the price was fair and reasonable. I’d recommend this route for other parents who have students meeting sports physicals.

The process was easy enough unfortunately the school didn’t accept it so I had to go pay for an additional physical

We had just moved to a new state a week before the start of school and were jumping through hoops to get the kids enrolled and get records and paperwork together. I contacted Sportslip support to ask a couple questions about the medical forms and they reached back out right away. I booked later that day and had my daughter's sports form filled out and certified within the same day. It was a very straightforward process and I didn't have to leave home when I already have two million things to do :) Great experience, I would definitely recommend.

Needed a sports release for my son to attend and participate at the football game on Friday. Completed the SportSlip process the Monday prior (paid the extra $20 for the expedited service) and received the fully compliant docs from SportSlip Tuesday morning! Immediately processed with the school and my son was cleared for the Friday game that same day! TELLING EVERYONE HOW GREAT SPORTSLIP IS. Even if we aren't in a rush, this process was so much easier and less stressful than booking an in person appointment, then driving to the Dr. in town weeks later (if I'm lucky), waiting around etc etc etc. We will only be using SportSlip in the future for all my kiddos.
